Clinical to Community Connections

ASTHO’s efforts related to building clinical to community connections focus on strengthening collaboration and integration between payers, providers, and public health agencies. The Clinical to Community Connections team collaborates with public health and healthcare stakeholders to develop tools and provide technical assistance that allow public health agencies to maximize resources through cross-agency and cross-sector collaboration.

Community Health Workers

This webpage contains resources for state health agencies interested in telehealth implementation, expansion, and using telehealth to increase access to preventive and specialty care, address health disparities affecting vulnerable populations, and save costs. Read more »

Primary Care and Public Health

ASTHO's primary care portfolio examines key areas of the intersection between public health and primary care and addresses the role of states in primary care transformation. This work also supports primary care offices by offering training and technical assistance to state primary care offices, and resources for state health officials on access to care and primary care topics. Cross-Sector Partnerships to Address Social Determinants of Health

ASTHO is working to help states, with the leadership of their state health agencies, to address the social determinants that affect our health status on a population level as part of our daily lives. ASTHO’s resources on partnerships to address these factors include issue briefs, policy guides, and resource libraries. Read more »

Medicaid and Public Health Partnerships

ASTHO supports state health agencies by providing information and technical assistance to facilitate partnerships with Medicaid. These activities encompass a range of payment and delivery reform initiatives such as the State Innovation Models (SIM) Initiative and promoting collaborative opportunities between Medicaid and public health that will achieve the Triple Aim of better care, reducing costs, and improving population health outcomes.
